Do I Need To Prepare For My Initial Consultation?
Our office will send intake forms after you schedule your initial consultation. We ask that you complete these forms within 72 hours of your appointment. Timely completion will give us time to prepare for your appointment. If you have a food log, growth charts, labs, or supplements for your child, you may bring them to the appointment.
Do You Accept Health Insurance?
We accept several insurance carriers, including Cigna, Anthem, and United Healthcare. Not all insurance companies cover nutrition counseling. We encourage you to verify coverage with your insurance company. We also offer a self-pay rate that is due at the time of service.
