Not all insurance plans cover nutrition, but many do. We encourage families to call their insurance company and ask if they have benefits for nutrition counseling, CPT code 97802/97803. Ask if there are any restricted diagnoses, if you have a deductible to meet before insurance coverage and if a referral is required. Ultimately, it’s up to the client to know their plan’s benefits and be responsible for payment. Click here for our instruction sheet on verifying benefits. Notice: Your Right to a Good Faith Estimate.
Nutrition Therapy Fees:
If you want NWPNF Nutrition to bill your insurance, they will be billed at the contracted rate. You will be responsible for the payment if this rate goes towards the deductible.
Self-pay rate (at the time of service, 30-40% below contracted rate):
Initial Consultation/Virtual/Office (60 minutes): $200
At this session, we will review medical records and intake forms received, conduct a more detailed interview to get to know you or your child’s nutrition needs, and provide initial recommendations and send assessments to medical providers.
Family Session (75 minutes): $225
This session is for a family where two or more individuals in the family have similar nutrition goals they want to discuss. Book this session if everyone lives together and eats most of the meals together.
Follow-up Consultations:
- Regular Follow-up: 50-55 minutes: $150
- Follow-up Brief: 25-28 minutes: $80 (Reserved for established clients that require more frequent shorter visits).
Packages:
When your insurance plan doesn’t cover nutrition or the current diagnosis isn’t covered, we want to offer you a package discount to make it easier to receive nutrition support.
-
Picky Eater Package: This is a package that includes the initial nutrition consult and three (55 minute each) Follow-ups that can be split up as the dietitian feels appropriate. $520 (20% discount)
-
Eating Disorder/ARFID/Intuitive Eating Package: This is a package that includes the initial nutrition consultation and seven (55 min) follow up visits that can be split up as the dietitian feels appropriate. $999 (20% discount).
Most with Eating disorders and ARFID will need more than 8 sessions, so additional packages of 5 follow ups can be purchased for a 20% discount as needed.
Policies:
- All fees are payable on the day of the session unless other arrangements have been made. A credit card will be held on file and charged on the day of service.
- All packages must be paid in full. Any requested refunds for packages will be refunded based off of self-pay rate. The 20% discount will no longer apply.
- If we are billing insurance and the claim goes toward the deductible, the client is responsible for the rate that goes toward the deductible.
- For an appointment canceled less than 48 hours in advance, there is a $50 fee not payable by insurance, which must be paid before scheduling your next appointment. For appointments canceled on the day of the appointment, there is a $100 fee. No-show appointments we charge a $150 fee. This policy is in place to ensure that clients needing to be seen can be seen since it is difficult to fill spots with short notice and that the dietitians on the team have a reliable income since we can’t bill insurance for a late cancel.
- NW Pediatric and Family Nutrition follows the Portland Public School snow day policy. If school is closed, NW Pediatric and Family Nutrition (Portland location) will be closed, and appointments will be rescheduled or will be virtual unless other arrangements have been made.
